What is Recuperative Care?
Recuperative Care is a program designed to assist Minnesota residents experiencing homelessness. It’s main objective is to prevent re-hospitalizations and provide medical care for those who are unhoused, in a shelter, or otherwise lacking stable housing. Program participants must also be ineligible for hospital-level care, skilled nursing care, or do not meet the severity of illness for other levels of care.
At East Side Communities, we prioritize a patient-centered approach focused on healing and recovery from illnesses. We offer essential behavioral health support, care coordination, housing solutions, and a comprehensive range of social services to ensure our patients’ successful recovery. Our team of dedicated healthcare professionals are committed to providing essential nursing care, medication management, wound care, and much more. This approach ensures a holistic perspective on health and well-being for every patient we serve.
To be eligible for Recuperative Care Services, individuals must meet the following criteria:
21 years or older if they have Medical Assistance, or 19 years or older if they have MinnesotaCare.
Experiencing homelessness or are unhoused.
In need of short-term medical care for fewer than 60 days.

Recuperative Care accommodations include:
Private bedrooms
Three meals a day
Reasonable modifications for special dietary needs
Access to laundry
Access to round the clock staffing
