Our Mission
Our mission is to help eligible Minnesota Health Care Programs (MHCP) members, those experiencing homelessness or living in unstable housing, recover safely outside the hospital setting.
Our Approach
We work in close partnership with local hospitals, clinics, and health providers to ensure that every member receives the care and support they need during their recovery.
